What is Transcranial Magnetic Stimulation?




Transcranial Magnetic Stimulation (TMS), is a non-invasive method for studying and treating brain conditions using electromagnetic pulses through the head. Originally invented in 1985 as a way to assess the effect of electric stimulation on cortex functioning, TMS has evolved through massive innovation to become an effective therapy for a variety of conditions.

Neuropsychologists, like ours at Absolute Bliss, provide a service called Repetitive TMS (rTMS for short). Through rTMS, a patient will receive multiple sessions of electromagnetic stimulation, focused on different parts of the brain cortex. By altering some different parameters—like stimulation placement and intensity—the patient can received localized treatment in the cortex where needed. The entire process is completely painless and safe, and has been praised by the medical community for it’s effectiveness.



HOW DOES TMS WORK, EXACTLY?
In ‘science talk’ TMS works by using a transformer to charge a big capacitor that is then discharged through the cranium. But let’s bring it into simpler terms, shall we?

Essentially, the equipment sends an electric pulse through a coil to the skull and into the brain. Once that pulse is in the brain, it stimulates the nerve cells located there. In doing so, this changes and excites the brain activities for a few short seconds at a time, which helps to create change and understanding of the brain functions.

WHAT DOES TMS TREAT?
The uses of rTMS are wide and varied, but there are a few areas where the benefits have been clinically proven.

Depression: Depression is a result of several brain irregularities, which results in a reduced cognitive control of emotion. Through rTMS, neuropsychologists can repeatedly stimulate the left DLPFC (a brain part) of a patient, which improves mood and helps restore normal brain processing. Several studies have been conducted to confirm these findings. And for even more evidence, rTMS was used effectively to treat depressed patients who were resistant to pharmaceutical approaches.

Anxiety: Generalized social anxiety disorder (SAD) occurs when there is a top-down imbalance in the brain circuitry between the amygdala and other prefrontal brain areas. With rTMS, we can target those areas and stimulate the connection where it is missing. 

Hallucinations (Schizophrenia): In the event of hallucinations, the speech recognition part of the brain is hyperactive. Through repeated stimulation of that brain part (known as Wernicke’s Area), doctors have been able to regulate the activity and reduce the intensity, thereby providing relief from hallucinations. 

Migraines: In 2013, rTMS received FDA clearance as a treatment for migraines, by stimulating the part of the brain where the pain is localized.

Stroke: After the brain has been damaged through a stroke, rTMS can be used to stimulate specific regions to speed recovery.

OCD: Obsessive compulsive disorder is a syndrome where patients have an overstimulated orbitofrontal cortex, and must have things be “perfect” for them to progress with their day. The rTMS treatment effectively lessens the stimulation of that part of the brain, leading to fewer unwanted thoughts and compulsions.
